APPLICATION REVIEW The KAN-ED Application for Consortium Membership has two basic sections. Section 1 - Your Library In this section you will enter: Library name Mailing Address Your e-mail address Phone Number Administrative Contact [Library Director] Technical Contact [If you have one] Director signs and dates. Section 2 - Internet Service Provider (ISP) Section Write in the name of your ISP Fill in the remaining information if you know what it is. If your ISP is not able to or unwilling to sign the form, send it to KAN-ED anyway. If you are a library with dial-up access only, please note that on your application and submit your form without the service provider signature. Mail Completed Applications to: KAN-ED
